The dentist role of jaw expansion is only one piece of the puzzle of health

When this patient first visited our practice, they were dealing with severe dental crowding and impacted permanent canines. Additionally, they had a pronounced downward growth pattern, resulting in a noticeable gummy smile.
We decided on a treatment plan that combined airway appliances and braces. The airway appliances played a crucial role in developing the patient's dental structure forward, which widened their smile and ensured enough space for all the permanent teeth to fit naturally.
After the appliances had set the groundwork, we transitioned the patient to braces. This final stage aligned all their teeth perfectly, achieving both functional balance and aesthetic improvement.
The outcome is an esthetically pleasing smile and enhanced airway space, which improved the patient's quality of life. The patient and their family are thrilled with the results, enjoying both the visual transformation and the significant health benefits.

When this patient first started treatment they have a significant downward pattern of growth, you can see the lower jaw rolling back. They also have major crowding on both the upper and lower arches. After treatment (on right) facial balance is much better. We were able to develop both the upper and lower jaws forward achieving an esthetically pleasing result, creating space for all permanent teeth, as well as a better jaw position for airway and function
Shows a significant improvement in airway volume
Which airway would you rather breathe through!

Perspective of Dr. Stanley . Liu MD, DDS, FACS - Sleep
“Do you find that the improvement in Breathing and Airway shown with DISE with maxillary expansion comes from the change of the muscles attached to the hard tissue or the improvement of the space for the tongue?”

The Patient was seen in the Sleep Disorders Program today, 10/21/2025, sleep study performed.
OHSU Sleep Disorders Program
Diagnostic Polysomnography (PSG) Interpretation
Date of Study: 10/21/2025
Clinical Presentation: The patient is a 2 y.o. female with retrognathia with current palatal expander (Dr. Tanner- airway dentist), normal tonsils (per parent this was reported by a community ENT provider they saw), sleep disordered breathing, behavioral insomnia (parent presence, bottles), restless sleeper, who underwent diagnostic polysomnogram to evaluate for sleep apnea.
Impression: Negative for obstructive sleep apnea, with an obstructive apnea hypopnea index of 0.6 events per hour. The majority of the respiratory events are obstructive hypopneas associated with REM supine sleep. Central apnea index is normal (<5/hr). Per tech notes, intermittent snoring noted and patient comforted by bottles during the night. Oxygen desaturation index is 0.1/hr (normal), oxygen nadir is 94% (normal), and mean oxygen is 99% (normal). EtCO2 is not consistent with sleep related hypoventilation (if anything, recorded values are a bit low and likely reflect cannula placement due to patient intolerance). Single lead EKG is normal. Overall sleep efficiency is normal and sleep architecture is normal. No obvious abnormalities on EEG. The patient has a normal number of periodic leg movements during sleep. Patient tolerated sensors well with the exception of the nasal cannula, but thermistor signal was clear during the night and normal oximetry which is reassuring against severe sleep disordered breathing.
